Lincolnshire Community Health Services NHS Trust is seeking to notify the market of an upcoming opportunity relating to the provision of short-term bed-based Transitional Care services in Lincolnshire.

The service will support timely discharge from acute hospital settings by providing time-limited care for individuals who no longer require acute treatment but are not yet ready to return home.
